This feature is located on the Draw ribbon. Click Format Background and select one of the lined paper option. I'm using College Ruled.

Report abuse. Instead of inserting it as a background or watermark, open the header and insert the picture there, formatted as Behind Text. Stretch the picture as needed to fill the page and set its position as Top and Left Relative to Page. You'll have to adjust the line spacing of your paragraphs to match the line spacing of your picture.
